How does the macro need change at older age?

As the energy need falls, protein's share rises. The difference between the measured intake and the recommended range at older age.

At older age the balance of the macronutrients changes in two directions at once. Because energy expenditure falls, the total energy need drops; against this protein's share in energy rises: the lower limit of the reference range, which is 10-20 percent in adulthood, goes up to 12 percent after 65. On the fat side Türkiye Beslenme Rehberi (TÜBER), Türkiye's national dietary guideline, gives a narrower ceiling for older people: at most 30 percent of daily energy should be supplied from fat.

Why does protein come forward?

The guideline states that taking in enough protein at every meal matters in preventing the loss of muscle mass seen in the older years, and asks that protein intake in particular be followed. The bioavailability of animal-based protein is higher and it is effective in protecting muscle mass.

There is a balance problem here. The saturated fat and cholesterol content of foods rich in animal protein is generally high as well. The guideline's solution is to go into detail in the choice: the leaner meats should be preferred, fish, chicken and red meat should be eaten in rotation, and balanced grain and dried legume mixtures should be chosen.

What does the measured intake say?

The guideline gives the real intake of the older age group with numbers as well. Daily energy intake is on average 1729.6 calories in men and 1351.3 calories in women. In men 15.1 percent of dietary energy was supplied from protein, 33.6 percent from fat, 11.4 percent from saturated fatty acids, 51 percent from carbohydrate and 7.1 percent from sucrose.

In women the same order is 15.1 percent, 34.4 percent, 11.6 percent, 50.5 percent and 7.4 percent; the two tables are close to each other. When these two values are read together, protein intake can be worked out in grams as well: about 65 grams a day in men and about 51 in women.

Which item is outside the range?

The guideline's judgment is clear: although the share of energy supplied from saturated fat is above 10 percent, the shares supplied from carbohydrate and from protein are within the recommended range. So the problem is not in the total distribution but in the composition within the fat itself.

Two more items are outside the range. More than 5 percent of the energy older people take in with the diet is supplied from sucrose, and fiber intake is insufficient: 22.9 grams a day in men and 18.6 grams in women. Neither catches the 25 grams the reference values table gives for this age group.

Practical recommendations

On the fat side the guideline takes visible and invisible fat together. A rise in the consumption of saturated animal fats such as butter and tail fat poses a risk; when meat, chicken, milk and cheese are eaten a lot, fat intake rises and most of it is saturated fat. In dishes and salads plant liquid oils such as olive oil, sunflower oil and corn oil are preferred.

Fish comes forward as a separate item: because of its omega-3 fatty acid content it should be eaten by older people at least twice a week. On the fiber side the foods with the highest content are dried legumes together with vegetables and fruit.

What changes on the carbohydrate side?

The energy share of carbohydrate stays in the 45-60 percent range at older age too, and the measured intake is inside that range as well. What changes is which foods fill that share. The guideline asks that whole grain bread and other grains be eaten in sufficient amounts and that sugar consumption be lowered.

The place of grains in this period is defined too: they are generally low in fat, they contain no cholesterol, they carry many minerals and they provide fiber. With these properties whole grains and bran products open a heading of their own in older age nutrition.

The target on the fiber side does not change. The reference values table gives 25 grams of fiber a day for men and women aged 65-70 and over; because the measured intake stays below this, the guideline lists raising fiber consumption separately.

Where does it turn into a mistake?

The first mistake is reading the falling energy need as lowering protein intake as well; while energy falls, protein's share rises. The second mistake is leaning only on red meat for protein; the guideline asks that fish, chicken and red meat be eaten in rotation. The third mistake is taking the measured values for a target: 1729.6 and 1351.3 calories show what is eaten in Türkiye, not what should be eaten.

The guideline's general frame in this section is short: the fall in energy expenditure that comes with ageing should be balanced by a fall in energy intake. Low fat milk products, eggs and dried legumes are counted as good quality protein sources.
